The Health Services Department at Fabens ISD will take every necessary precaution to make sure that your child's Diabetic needs are met. If your child has Diabetes, takes medication, has an insulin pump or will require carbohydrate counting, please take the "FISD Diabetes Action Plan" to your child's medical provider and have them fill it out and sign. If your child will be monitoring their own diabetes and/or will be allowed to carry their insulin or emergency medication, you and your child will need to fill out and sign the "Diabetes Self Management Agreement".
