The Health Services Department at Fabens ISD will make every possible arrangement to make sure that your child's medical and dietary needs are met. If they have a food allergy at risk for anaphylaxis and they will require special dietary arrangements, please fill out the "FISD Allergy Plan and the "Special Dietary Needs Request Form" and have it signed by your child's medical provider. If they will be allowed to carry their emergency medication, i.e. Epinephrine Pen, you will need to fill out the "FISD Allergy Plan" and have it signed by your child's medical provider and you and your child will need to fill out the "Self Management Agreement".
